You may view and print your form online through Paycor.com or you may request a paper copy.

If you wish to receive a paper copy of your Form 1095-C, you may request one at no cost. Your request will be fulfilled within 30 days. Please submit your request via phone (605-336-5000) or by mailing us at 140 E Hinks Ln, Sioux Falls, SD 57104.
